How EMFACE Works: Synchronized RF + HIFES, Explained
EMFACE works by running two technologies through the same applicator at the same time: radiofrequency that gently warms the skin to stimulate collagen and elastin, and HIFES stimulation that contracts the facial muscles far more intensely than you can voluntarily. Together they treat skin and muscle in one 20-minute session.
How does EMFACE actually work?
EMFACE places pads on the forehead and cheeks that deliver synchronized radiofrequency and HIFES together. The radiofrequency warms the skin layer; the HIFES energy makes the facial muscles contract. Doing both at once, in the same spot, is the core idea — BTL calls it "synchronized."
What does the radiofrequency do?
The radiofrequency raises the temperature of the skin in a controlled way. For aesthetic use, BTL states this is intended to stimulate collagen and elastin production — the proteins that give skin its firmness and bounce.
Radiofrequency energy has been used in aesthetics for years to heat the deeper skin and prompt a remodeling response over the following weeks. In EMFACE, that skin-warming happens at the same time as the muscle work — which is what sets it apart from radiofrequency-only devices that address the skin surface alone.
What is HIFES, and what does it do?
HIFES stands for High-Intensity Facial Electrical Stimulation. It makes the facial muscles contract selectively and repeatedly — far more than voluntary movement — which BTL describes as inducing "natural muscle activity" to condition and tone the muscles that support your features.
The muscles most studied are the frontalis (which lifts the brow) and the zygomaticus major (which lifts the midface when you smile). By stimulating these support muscles, EMFACE aims to improve their tone — the "lift" side of the treatment, as opposed to the skin-quality side handled by radiofrequency.
Why do both at the same time?
A face ages at more than one layer: the skin loosens and the support muscles lose tone. Treating only one layer leaves the other unaddressed. EMFACE’s premise is that synchronizing skin-directed radiofrequency with muscle-directed HIFES addresses both in a single, needle-free session.

What does the science show about the mechanism?
Peer-reviewed studies report measurable change in both layers: improved wrinkle scores (skin) and improved muscle "tone" on ultrasound at three months. A porcine histology study also reported structural muscle change. The evidence is early and manufacturer-linked, but it is consistent with the two-layer mechanism.
For the muscle side, the Halaas 2025 ultrasound pilot measured a roughly 31% improvement in muscle echo intensity at three months in the frontalis and zygomaticus. For the skin side, Robb 2025 measured about a 35% wrinkle-score improvement at three months. We note again that these studies are small and manufacturer-linked — real signals, early evidence.
Does the jaw version work the same way?
For its FDA-cleared TMJ use, the same technologies are aimed at the jaw muscles: radiofrequency to raise tissue temperature and support circulation, and HIFES to activate and re-educate the jaw muscles. See EMFACE for TMJ for how that applies to jaw tension.
Frequently asked questions
- No. EMFACE uses HIFES (High-Intensity Facial Electrical Stimulation) for the face. BTL’s body devices, such as Emsculpt, use a different technology called HIFEM. They are different systems for different areas — see our EMFACE vs Emsculpt guide.
- Most people notice a gentle warmth from the radiofrequency and a distinct tapping or contracting sensation from the HIFES. They happen at the same time. Published studies reported the experience as generally comfortable with low discomfort scores.
- It stimulates facial-muscle contractions more intensely than voluntary movement, and studies report improved muscle tone on ultrasound. It is reasonable to think of it as conditioning the support muscles — but results are individual and measured only to about three months so far.
- Radiofrequency-only devices treat the skin. EMFACE adds synchronized HIFES muscle stimulation, so it addresses the support muscles as well as the skin in the same session. That combination is the defining feature of the treatment.
