What Is EMFACE? A Clear, Honest Explainer
EMFACE is a non-invasive, needle-free facial treatment from BTL Aesthetics that uses two technologies at once — synchronized radiofrequency and HIFES muscle stimulation — to address the skin and the muscles beneath it in one 20-minute session. It is FDA-cleared, and a 2025 clearance added the treatment of TMJ dysfunction.
What is EMFACE, in plain terms?
EMFACE is an in-office, needle-free facial treatment made by BTL Aesthetics. It combines synchronized radiofrequency (which warms the skin) with HIFES muscle stimulation (which contracts the facial muscles), delivered together through pads placed on the face — no injections and no downtime.
Most facial treatments work on a single layer — either the skin surface or, in the case of injectables, a specific muscle or a pocket of volume. EMFACE is unusual because it treats two layers in the same session: the skin (with radiofrequency) and the muscles that support your features (with HIFES). BTL describes it as a therapy that works "both functionally and aesthetically."
How is EMFACE different from Botox, filler, or a facelift?
Unlike Botox (which relaxes a muscle) or filler (which adds volume), EMFACE tones and conditions the facial muscles while stimulating the skin. Unlike a facelift, it is non-surgical — nothing is cut, removed, or repositioned. It changes how your own tissue is toned, not its structure.
- Botox temporarily relaxes targeted muscles with a neurotoxin. EMFACE stimulates muscle activity instead of blocking it — see EMFACE vs Botox.
- Fillers add volume with an injected gel. EMFACE adds no volume — EMFACE vs fillers.
- A facelift surgically tightens and removes tissue for a lasting structural change. EMFACE is non-invasive with no recovery — EMFACE vs a facelift.
What can EMFACE realistically do — and not do?
EMFACE is designed to support firmer, more toned-looking skin and a subtly more lifted appearance in the forehead, cheeks, and jawline area. It is not a facelift and will not remove significant sagging or excess skin. Results are gradual, individual, and not permanent.
In peer-reviewed studies, EMFACE improved measured wrinkle scores and facial-muscle tone over about three months (details below). It is best understood as a way to refresh and tone — not to reverse advanced laxity, which is the territory of surgery. An honest consultation is the only way to know whether your goals are a realistic match.
Is EMFACE proven to work?
There is published, peer-reviewed evidence that EMFACE reduces measured wrinkles and improves facial-muscle tone at three months. Every study to date, however, has been funded by or authored with the manufacturer and used small groups (about 10–33 people), so the evidence base is early.
In a 2025 multicenter study of 33 people, Robb and colleagues measured an average wrinkle-score improvement of about 35% at three months using 3D analysis, with most participants satisfied. A separate 2025 ultrasound pilot by Halaas found facial-muscle "echo intensity" improved about 31% at three months, an objective sign of better muscle tone. We think it is important to say plainly that these studies are manufacturer-linked and small — promising, but not the same as large, independent trials.
EMFACE and TMJ: the newer use
In August 2025, EMFACE was FDA-cleared for the treatment of TMJ dysfunction and associated pain, muscle re-education, increased local blood flow, and maintenance or improvement of jaw range of motion. That makes the same device relevant to jaw tension — not just appearance.
This is where our practice is unusual: Dr. Kate is a neuromuscular dentist who treats the jaw and its muscles every day, and an EMFACE provider. If jaw tension or clenching is part of your picture, see EMFACE for TMJ. Note the precise wording — EMFACE is FDA-cleared (not "approved") for this use, and we stay inside that cleared language.
Who provides EMFACE at iDental Dilworth?
EMFACE at our Charlotte practice is provided under Dr. Kathryn Sudikoff, DMD, an LVI Fellow with years of postgraduate training in neuromuscular dentistry. Because she works with facial muscles and the jaw daily, EMFACE fits naturally alongside her cosmetic and TMJ care.
The honest version
EMFACE is a legitimate, FDA-cleared treatment with early but real evidence — not a miracle, and not a facelift. If someone promises guaranteed or permanent results, be cautious. We would rather tell you candidly whether it fits.
Frequently asked questions
- No. EMFACE is a non-invasive treatment that tones facial muscles and stimulates the skin — nothing is cut, removed, or surgically repositioned. It is not a substitute for a surgical facelift and does not treat significant sagging or excess skin.
- Yes. EMFACE is FDA-cleared for aesthetic facial use, and as of August 2025 it is also FDA-cleared for the treatment of TMJ dysfunction. "Cleared" is the correct regulatory term for this type of device — not "approved."
- Most people describe it as comfortable — a warming sensation from the radiofrequency plus a tapping or contracting feeling from the muscle stimulation. In published studies, average discomfort scores were low and no downtime was required.
- The standard protocol is four 20-minute sessions spaced about a week apart, which is what the clinical studies used. Your provider may recommend periodic maintenance afterward. Your exact plan is confirmed at consultation.
- No. Results build gradually and were measured in studies out to about three months. They are not permanent, and clinicians typically recommend periodic maintenance to help sustain them. How long they last varies from person to person.
